By completing this Release, I, ______________________________ (“Customer”), hereby grant MindCast LLC (“Company”) permission to use my written testimonial.
This testimonial may come from FundaMENTALs Module Completion Feedback, Email, social media correspondence, the 40-item well-being measure, the Flourishing Scale, or any other written, video, media, or visual material directed towards MindCast LLC or Know Your FundaMENTALs. I understand that my testimonial may be published, distributed, and used in connection with promoting the Company’s coaching programs and/or business.
By submitting this form, you are agreeing to the following:

Permission to Use:
I give permission to Kelly Berthold, LCSW, and MindCast LLC to use my testimonial (written, audio, or video) — in whole or in part — for marketing, educational, and promotional purposes, with no expiration date. -
How It May Be Used:
My testimonial may be used on websites, social media, email newsletters, printed materials, presentations, or any other media or format related to MindCast LLC or Know Your FundaMENTALs. -
No Compensation or Obligation:
I understand that I will not receive monetary compensation for the use of my testimonial and that participation is voluntary. -
Right to Withdraw:
Although this permission is granted without time limit, I understand that I may withdraw my consent at any time by contacting Kelly Berthold, LCSW, at kelly@knowyourfundamentals.com. Upon request, any future use of my testimonial will be discontinued. -
Acknowledgment:
I confirm that my testimonial represents my honest experience and that I am at least 18 years old.
I irrevocably authorize the Company, as well as its affiliates, employees, agents, licensees, or any other party designated by the Company, the unlimited right to copy, publish, distribute, edit, and use my testimonial. I agree that I will not seek any payment or make any claim against the Company for its use of my testimonial.
I hereby hold harmless and release MindCast LLC and Kelly Berthold from any causes of action, claims, or demands that I or anyone acting on my behalf may have regarding this authorization.'
By signing and dating the FundaMENTALs Module Completion Feedback and Journal Reflection optional section: Testimonial & Photo Release (Optional) in the FundaMENTALs Life, Relationships and Career Accelerator course, or as discussed in the coaching program agreement, I have read, understood, and consent to the above.
